SovrenMD may operate in different roles depending on the service involved. In some circumstances, clinical services are provided by licensed physicians, nurse practitioners, physician assistants, clinics, hospitals, or other independent provider entities. SovrenMD may also provide non-clinical support services such as intake support, scheduling, care coordination, device logistics, patient summaries, communications support, operational workflows, and administrative services. Where required by law, we handle protected health information in accordance with federal and applicable state privacy requirements.

We may use and disclose your health information to provide, coordinate, or support your care and to share information with clinicians, partner providers, pharmacies, laboratories, imaging centers, remote monitoring teams, and other healthcare professionals involved in your care.
We may use and disclose your health information to bill for services, collect payment, verify eligibility, respond to payor requests, process claims, coordinate benefits, and manage accounts.
We may use and disclose your health information for healthcare operations, including quality review, compliance, auditing, care coordination, training, credentialing support, service improvement, customer support, and business administration.
We may contact you regarding appointments, telehealth sessions, onboarding, device setup, follow-up care, monitoring support, billing matters, and other service-related communications.
Unless you object, or where otherwise permitted by law, we may share relevant information with family members, caregivers, guardians, or personal representatives involved in your care or payment for care.
We may use or disclose your information when required by federal, state, or local law, court order, subpoena, legal process, public health reporting obligations, or lawful government request.
We may disclose information when legally permitted or required for public health activities, abuse or neglect reporting, health oversight, or to prevent or lessen a serious threat to health or safety.
We may disclose information to vendors and contractors that support our operations, including telehealth platforms, secure communication vendors, cloud hosting providers, billing support vendors, device logistics vendors, and related service providers, where appropriate safeguards are in place.
Except where otherwise allowed by law, we will obtain your written authorization before using or disclosing your health information for purposes that require authorization. You may revoke an authorization in writing, except to the extent action has already been taken in reliance on it.
Certain categories of information may receive additional protection under federal or state law, including substance use disorder treatment records, certain mental health records, HIV-related information, genetic information, reproductive health information, and records involving minors. Where those laws apply, we will follow the additional protections required.
